Every DPC practice in our network runs a true direct-care model: real providers, panel sizes under 600 patients, and same-day access. We've spent months building this network so you don't have to go find it yourself.

Direct Primary Care

A local, independent provider, not a network, not a clinic chain. Employees get same-day or next-day appointments, direct phone and text access to their provider, and zero copays per visit. Panel sizes of 400 to 600 patients instead of the 2,000+ typical in insurance-based practices. Your provider knows your employees by name.

24/7 Virtual Care

Virtual primary care for ongoing conditions, virtual urgent care for after-hours symptoms, and virtual counseling for mental health support. When a referral is needed, specialist and diagnostic navigation helps find and compare pricing before your employee ever steps into an office. Available on any device, no appointment required for urgent needs, no copay.

Prescription Savings & Labs

Prescription savings cut most everyday medications well below retail, often for less than a typical copay. Labs are covered in full when ordered by your virtual care team. Vision and dental access available at reduced rates. Full drug list at meds.elevateinsured.com.

Optional coverage, employee-funded

Hospital indemnity, critical illness, and accident coverage are available to employees who want them. They pay for it; you don't. A licensed Elevate Insured agent handles enrollment directly with each employee. No additional employer cost, no administration on your end.

We set it up

We negotiate the employer group rate with your local DPC partner, handle all enrollment paperwork, and onboard your employees. Most Phoenix-area teams are live within two weeks, with benefits typically starting the 1st of the following month.

It runs

No open enrollment window. Employees can join at any time. No minimum participation requirement. We handle administration. You get one invoice per month and a direct line to us if anything comes up.

Is there a contract?

No long-term contract required. We ask for a 90-day minimum commitment to get everything set up and running for your team, then it's a straightforward month-to-month agreement with 30 days' notice to cancel.

How does coverage work for spouses and dependents?

That's entirely up to you. You can choose to cover all employees' family members, only children, some employees and not others, or none at all. Anything you choose not to cover gets billed directly to the employee through their portal, and adding family members is generally available at a discounted membership rate.
